General Information About Online Forms

Forms make take one of three formats:


  • A regular form you fill in and submit.
  • A PDF form which you print out, complete, and mail.
  • A Microsoft Word document.
      Please note, a browser plug-in is required to read a PDF document. You can download the Adobe Acrobat Reader for free if you don't already have it. Typically, a document requiring an exact format would be furnished as a PDF document. An example might be an Employment Application form. Print a copy of the document once it is displayed by your Web browser. The form will contain specific instructions for completion and submission.
